Community Health networking with the stakeholders

Intro

This practice is related to ComSalut Project (Community and Health) led by the Public Health Agency of Catalonia which select 16 Primary Health Center to develop the project. We explain the experience of Nova Lloreda Primary Health Center, managed by Badalona Serveis Assistencials, a public organization that offers health services, with 16 centers and more than a thousand professionals. The aim of this project is promote community health across networking between Public Health of Catalonia, City Council of Badalona, Primary Health Care Team Nova Lloreda, and Civil Society, developing actions with the local assets to improve population’s health and wellbeing.

Objective of the practice

The project started on 2016 with working groups to identify health assets in the territory and elaborate the health diagnosis, according to quantitative and qualitative data, positive and negative aspects, needs in the community, and prioritizing actions to implement. 
 
Also we collected 169 opinions from citizens by a questionnaire. The working groups detected 5 main problems: loneliness, family overload in elderly people, mental health, unemployment, lack of motivation in young people. The health diagnosis was concluded in 5 meetings. In the last meetiong 57 Citizens selected 2 priorities: Mental Health and Loneliness and family overload in elderly people. 
 
The working groups were composed by third sector organizations, community stakeholders as pharmacies, schools, patients associations, neighborhoods associations, association of young people, mental health association, elderly people center, Library, local City Council, technicians of Public Health of Catalonia, healthcare professionals, primary care team. 
 
This project improves coordination and promotes synergies on healthcare and welfare.

Partners

Key Stakeholders: since 2008 the Primary Health Center Nova Lloreda keeps relationships with its stakeholders as pharmacies, schools, patients associations, neighborhoods associations, association of young people, elderly people center, Library, local City Council. They have at least 3 meetings during the year. This is one of the reasons why the Center was selected to participate in this project. The project allows to detect new health assets in the territory.

Implementation of the Project/Activity

Concerning Mental Health several health assets have organized 8 conferences from October to December 2018 about emotions, physical exercise to prevent depress, relaxing techniques, drug addition, children education and young well-being. Health care professionals have also participated in some of these lectures. In each session we asked attendance to answer a questionnaire in order to assess their opinion about the lecture and other subjects of interest.

From February to June 2019 we have prepared 5 news conferences with local assets in the territory about active aging, physic exercise and children's education.

Concerning to Elderly People Project 16 volunteers have been trained by a social educator to detect elderly people at risk of loneliness. They contact to the Primary Care Team Nova Lloreda in order to decide the best resource for the situation: a health resource or a social resource. This project started on October 2018.

Results/Outputs/Impacts

Concerning the Mental Health Project, 137 people have attended the conferences organized from October to December 2018. We are analysing their opinion about the lectures according to the questionnaire they have answered.

Stakeholders are interesting in continuing on this project, and for this reason we have prepared 5 news lectures from February to June 2019.

Concerning Elderly People Project it started on October 2018. The Primary Health Centre has received 3 telephone calls and the team has evaluated the situation.

Enabling factors and constraints

The support of Public Health of Catalonia, the City Council of Badalona, the collaboration of local assets of the territory, and the Primary Care Team have contribute the succeed of the practice.

As a specific constraint the quantitative data for the health diagnosis of the territory and increase citizens participation.

Sustainability and replicability

Concerning the Mental Health project by the moment we have prepared 5 news conferences from February to June 2019.

Besides, since February 2019 the Primary Health Center offers a new service in Psychology to assist patients.

The Elderly People project is active in order to detect new situations.

The working groups are consolidated and they are 30 entities which participate in the Mental Health projects and in Elderly People group.

Badalona Serveis Assistencials is working with the local health assets in the community in the other 5 primary health care center of the organization, mapping the local assets in order to elaborate the health diagnosis of the territory and carry out actions to improve health and wellbeing.

We will continue the development of activities with the stakeholders in order to improve population’s health and welfare .

Conclusions

Networking with the stakeholders is very appreciated in the community as it promotes synergies. Citizens are protagonist of their own health. 
 
This project contributes to health promotion, establishing actions based on their needs, activating participation of the health assets in the territory and developing actions together to improve population’s health and wellbeing.
