Strengthening Public Health Services through Evidence-based Practices for saving Maternal & Newborn Lives in Remote Tribal Areas of Chhota Udepur District of Gujarat, India

    Description
    Intro

    Chhota Udepur district has tribal population with very high infant and maternal mortalities. Deepak Foundation established CEmONC centre at CHC-Jabugam, Chhota Udepur for providing maternal and child health services. The goals of the project are to reduce Maternal Mortality Ratio to less than 100/1,00,000 live births and Infant Mortality Rate to less than 30/1000 live births. The goals are set in line with Sustainable Development Goal 3 (SDG, 2015), the goals of National Rural Health Mission (NRHM, 2005) and the priorities of Gujarat Population Policy (2002). Beneficiaries include pregnant and nursing women and infants. The project achieves the goal of Universal health coverage (UHC).

    Implementation of the Project/Activity

    a) Process of Implementation of the project Identification and capacity building of grassroot functionaries was done so as to deliver safe motherhood and neonatal care services in the communities. Behaviour Change Communication activities were conducted in order to raise awareness regarding danger signs during pregnancy and post- delivery and provision of timely referral services. Emergency Transport facility was established for referral of complicated cases requiring obstetric and new-born care. Strengthening of Government Health Delivery System was done and demand was created for maternal and child health services. Community was mobilized to seek services with the help of village health functionaries (ASHAs, AWWs). b) Reforms made in the project as per need of the communities and our work experience Since the peripheral health centres of the government were not well equipped to conduct institutional deliveries, it was decided to activate Primary Health Centre’s and Sub-Centre’s as a part of the project. During the project implementation, verbal autopsy reports revealed that high percentage of maternal deaths are due to anaemia. Hence, a component of anaemia control was added as a part of the project. When the patients are referred to a tertiary health centre-SSG Hospital, Deepak Foundation found out that there is a lack of reception centre which can guide and support patients and their family to avail the required health services. Hence, Deepak Foundation started Help Desk at SSG Hospital. Since the tribal blocks lacked connection of roads and good transport to reach the nearest hospital, Deepak Foundation initiated Mobile Health Units (MHUs) to reach out to the tribal population who were not able to approach the nearest peripheral health facility.

    Results/Outputs/Impacts

    Total OPD beneficiaries (from April 2006- Feb 2021) are 446948 Total ANC beneficiaries(from April 2006- Feb 2021) are 242389 Total PNC beneficiaries(from April 2006- Feb 2021) are 26723 Total Pediatric beneficiaries(from April 2006- Feb 2021) are 88145 Total IPD conducted(from April 2006- Feb 2021) are 101804 Total deliveries conducted(from April 2006- Feb 2021) are 39119 Total surgeries conducted(from April 2006- Feb 2021) are 5177 On an average, total number of maternal lives save per year is 1700 and total number of infant lives saved per year is 1750

    Enabling factors and constraints

    The enabling factors are: Partnership with Government-The project is an innovative PPP model with shared resources. Robust Management Information System (MIS)-Web based Computerized Management Information System is set up to record and track health indicators of beneficiaries. The constraints are-Delay in receiving grant from Government- Since the project gradually increased to a 90:10 PPP model in which 90% of the total expenses are borne by the Government; there have been occasions in which the Government grant was not received on time. Staff attrition and retention- Since the geography of the project is tribal, the staff attrition is more.

    Sustainability and replicability

    Sustainability – The project is a sustainable model as the government sharing was gradually brought to 90% of the total expenditure. CEmONC is one of a unique and innovative PPP model that not only achieves the goals of Universal health Coverage but also has a ripple effect in improving utilization of government health services.The engagement in the model more than a decade has set up systems and procedures that could be utilized for quick and assured results which can be easily replicated in other tribal areas. The model also provides opportunity for shared resources and partnerships between government, corporate partners and e Civil Society Organizations to achieve the goals of SDG. Replicability- The project has been replicated in Jobat, Alirajpur district, Madhya Pradesh from 2016-2018 in partnership with Government of Madhya Pradesh. The project has also been replicated in Dumri, Giridh district, Jharkhand from 2017-2020 in partnership with Government of Jharkhand.

    COVID-19 Impact

    Due to COVID lockdown, the transport services were shut down completely thus reducing OPDs and deliveries as the services were not accessible to the communities who were residing peripherally in hard to reach/interior areas. But gradually after lockdown, the services resumed smoothly.

    The beneficiaries include pregnant and nursing women and infants. The key stakeholders of this project are Department of Health and Family Welfare (DOHFW), Government of Gujarat and Deepak Nitrite Limited. Stakeholders from the government include Regional Deputy Director, CDHO, Medical Superintendent and THO. The project is being operationalized on a Public Private Partnership model in partnership between DoHFW, GoG and DNL
